Choose the Option That Works Best for You:

• Option 1: Fill Out at the Clinic Prefer to handle it in person? No problem at all! We’ll have all the necessary forms waiting for you when you arrive. If you choose this option, we just ask that you arrive about 15 minutes before your scheduled time. This ensures you have plenty of time to relax, enjoy a cup of coffee, and finish your paperwork without feeling rushed before your evaluation begins.

• Option 2: Fill Out at Home. If you’d like to save time and settle in from the comfort of your own couch, you can download and print our new patient forms right here  Simply fill them out at your convenience and bring them with you to your first appointment. This gives our team a head start on reviewing your history before you even walk through the door!
